I just realized that, for the most part, I think I focus my eyes
behind the golf ball when making contact. Where SHOULD a golfer be
focusing when making contact? Is it just personal preference? Front,
middle or back of golf ball?

Question two: when using a 3 Iron, typically where should the ball be
played in the stance? Assuming lie is flat and you just want to hit a
straight shot with normal height for a 3 Iron.

If I focus on the ball at all, I'm looking at the back part of it. But
when I'm swinging, I'm usually not focusing on the ball; I'm focusing on
making a good swing, and thinking of the target.

3-iron: a bit back from the left heel. Try variations of that and
you'll find the exact spot you should hit your 3-iron from. (and I'm
not being flippant here--the best position depends on your club and your
swing. Different swingweight irons will produce different "best
positions" so you have to experiment a bit).

I suspect that it's just personal preference and that it is simply far
more important to be consistent than to pick the "correct" place to lok.

You want to hit long irons with a strike that is just a little on the
downswing. So take some practice swings that just brush the grass, and
place the ball a couple of inches behind where the centre of the area
you brush is.

I wish I could help, but I dont really remmeber hitting the golf ball. When
I initiate my downswing everything goes blank. I feel and hear it, but
thats it. I dont know what its like visually.

It's called "downstroke blackout".
